How the work is done
Methodology
What the machine does
- Every page is read, ordered by service date, indexed and Bates-stamped.
- A page that duplicates a kept page is set aside reversibly; a borderline page is kept and flagged.
- A fact is admitted only if its words appear in text quoted from the page it cites.
- A statement that cannot be anchored is withheld and reported, never delivered.
- Pages the production never delivered are read from its own stamps and counted. The count is proven, never guessed.
- Pages that carry no readable text are listed as unreadable.
What a reviewer does
- A person opens entries against the pages they cite before the bundle ships.
- Every review action is in the audit log that ships with the bundle.
- The reviewer’s location is printed on the verify sheet, read from the reviewer record.
- No reviewer is ever named to a client.
What is never done
- No narrative, no summary, and no opinion on cause, merit or value.
- No dose is converted, no drug is renamed, and no line is translated.
- No records are requested from a provider.
- Nothing here is legal or medical advice.
The legends, as they print
Every document carries the legend for what it is. These are those legends, word for word.
Clerical organization only - for attorney/administrator review. Not for use in QME/AME med-legal report preparation (Lab. Code § 4628).
Medical chronology prepared for attorney review. Every entry is quoted from and linked to its source page; entries are an index to the records, not a substitute for reading them, and contain no medical opinion. Not for use in QME/AME med-legal report preparation (Lab. Code § 4628).
Arithmetic only: days between the dates printed on the pages. States no cause and no opinion.
Copied as printed: the drug, dose and route are quoted from the line beside them. No dose is converted, no drug is renamed, no line is translated, and this is not a current medication list. States no cause and no opinion.
No page was removed without being text-identical to a kept page or confirmed by a reviewer.
This report states whether each citation is supported by the cited page. It states no medical opinion and no view on the merits.
A chronology, demand or medical summary you already have is checked against the records it cites.
Priced per page of the record set on the published tiers; the document being audited is not charged.
The four verdicts a citation audit prints
- SUPPORTED
- The cited page carries these words, quoted as the page prints them.
- SUPPORTED ON ANOTHER PAGE
- The words are on a different page of the set, and that page is named.
- NOT FOUND
- No page in this set carries these words. It is not a statement that the fact is untrue.
- UNREADABLE
- The cited page carried no readable text.
