How to find missing pages in a medical records production
A production testifies to its own completeness, twice. The findings are arithmetic on numbers the production already carries.
Check one: the stamp sequence
Most productions carry a sequential stamp on each page: a Bates number or a production stamp. If the stamps run PROD-000001 through PROD-000059, the numbering says fifty-nine sheets existed. If fifty-seven arrived, at least two did not.
Read every legible stamp, sort them, account for every number in the span. Three things complicate that, each in one direction:
- Pages whose stamp cannot be read. Each is subtracted from the claim before it is made.
- Repeated stamp readings. Two pages reading one number usually mean one misread. Treat both as unaccounted-for, never resolved by assumption.
- A break at the end of the sequence. A sequence that stops says nothing about what came after. Only interior gaps are evidence.
Check two: the documents’ own page numbering
Multi-page documents number themselves. “Page 3 of 5” in a footer testifies that pages 1, 2, 4 and 5 existed. If a five-page report contributes four pages, one page is absent, whatever the stamps say.
The two checks are independent: a page invisible to one is often plain to the other.
Why the only defensible finding is a lower bound
Both checks can prove a page is missing. Neither can prove how many. Pages with no legible stamp, single-page documents and unnumbered material can go missing without evidence.
The defensible wording is a floor: “At least two pages produced by the custodian did not arrive. The production’s stamps run PROD-000001 to PROD-000059 (59 sheets); 57 pages arrived, of which 54 carry a legible stamp.”
Every number in it is verifiable from the production itself.
What to send the custodian
A follow-up request needs three elements: the stamp range, the unaccounted-for numbers, and those pages. Ask in the alternative for written confirmation that they were never issued.
Doing this at scale
On a five-thousand-page production it is thousands of readings. One misread stamp invents a gap that was never there.
Claracord reads every stamp and every page-number footer, and nets out everything unreadable. Missing pages are reported as a floor, worded so the recipient can verify it by eye.
Human-verified. A complete sample, with the completeness report, is on the home page. Synthetic demo case.
